How does LEAP use the Key Worker Approach?

The team members at LEAP are known as Key Workers. They each have specific professional backgrounds and are highly skilled in all developmental areas. The Key Workers at LEAP share their knowledge amongst the team to ensure everyone has the most up to date skills. Our Key Workers include Early Childhood Teachers, Occupational therapists, Speech Pathologists and Physiotherapists.

How will we provide services to you and your child?

Our therapists and teachers work with you and your child in ways that are family-focussed, individualised and play based. The service we provide is informed by your goals. That is, your Key Worker will work on aspects of your child’s development that are meaningful to you and your family.

This might be in your home, in our centre, in school or preschool or out in the community.

First Step

Intake Appointment

Your first appointment will be an intake meeting with our Intake Officer. They will fill out any paperwork needed, get to know what services you might need and this helps us when allocating a Key Worker.

The intake meeting is not charged from your NDIS plan and usually takes about an hour. For this meeting, it is helpful to bring along your child’s NDIS plan, any assessments you have already and any other relevant information.

Next Steps

Initial sessions with your Key Worker

In getting to know you and our child, your Key Worker will complete a Routines Based Interview (RBI). This allows us to set some goals you’d like to work on, based on your daily life. At LEAP, we understand that children develop best when being supported in daily activities, and this is an important part of how we will work with you. The RBI helps us to understand what happens in your child’s day, the different activities they participate in, what they love to do, what they might find hard to do and any other things that happen in your family.

This session helps us to set realistic goals that are important to you and your child. This session can take up to 2 hours, so we might complete it over a couple of sessions.

Ongoing Sessions

Once your goals have been set, your ongoing sessions will start occurring. Your Key Worker will visit you and your child where it suits you most. This will be a discussion that you have with your Key Worker. As your child’s goals change and develop, the amount or frequency of session may change, based on discussions you have.

Consults

At LEAP, we work in a transdisciplinary key worker approach with children and families. This means that all Key Workers work together to share skills and knowledge and work together towards the same goals. For example, most Key Workers will use some Key Word Sign or visuals in their practice with all children.

An example of how this might work… If your child has a Speech Pathologist as their Key Worker, and one of their goals is working towards riding a bike, your key worker would initially have a secondary consult with our team physiotherapist or occupational therapist. They would discuss your child’s goal, some things that you could start with, and the physiotherapist might give your Key Worker some basic tips and tricks to get started on bike riding.

If the goal is more complex (e.g. assessing the need for a piece of specialised equipment), your Key Worker would organise a primary consult where the physiotherapist would come along to a session and meet with you and your child to commence some initial sessions.

These are just some of the ways the transdisciplinary key worker will support you and your child’s goals.
